Step 1 of 2: COMPLETE YOUR INTAKE FORMS

We require completing your intake paperwork within 24 hours of securing your intake date for injury management and 72 hours for neurologic/concussion cases so Dr. Burkhardt can review and prepare.

For neurological or concussion care, use the "Neuro" button. For treatment for any other injury or maintenance care, please use the "Standard" button.
