We require completing your intake paperwork within 24 hours of securing your intake date for injury management and 72 hours for neurologic/concussion cases so Dr. Burkhardt can review and prepare. If the doctor sees something that leads them to believe he can't help you, you are eligible for a full refund. We tried our best to limit the number of repeat questions: some purposeful redundancy is built-in for thoroughness. 

 

For neurological or concussion care, use the "Neuro" button. For treatment for any other injury or maintenance care, please use the "Standard" button.

Once we are confident that we are a good fit for each other, we will find a date that works for both your schedule and ours. Dr. Burkhardt has limited availability, so you must deposit 50% of your visit cost to hold the appointment. The deposit is fully refundable if your appointment is canceled within 72 hours of your appointment date. Please review our policies and schedule at the bottom of the page.

Please contact our team before scheduling for concussion management, as we will require additional information.

  • Our practice is appointment-only, with same-day bookings available for existing patients, subject to availability. These can be arranged online with a minimum of 2 hours' notice. Should you be unable to find an available slot for your preferred service, it indicates we are fully booked for the day.

  • New or returning patients who have not visited in over two years, or those presenting with a new injury, are required to select the "New Patient Exam" option when scheduling an appointment.

  • New patients must complete all necessary paperwork at least 24 hours prior to their appointment, or 72 hours in advance for neuro/concussion cases. Failure to complete this documentation will lead to the cancellation of your appointment and forfeiture of your deposit.

  • Punctuality is crucial; arrivals over 5 minutes late may result in appointment cancellation to ensure the quality of service and respect for other clients' time.

  • Our cancellation policy mandates notification at least 24 hours before your appointment, or 72 hours for neuro/concussion treatment appointments, to avoid a $50.00 cancellation fee or deduction from your treatment package. Changes can be managed through the link in your booking confirmation email or text.

  • Only the client and the doctor are permitted in the treatment room during appointments.

  • Payment is required at the time of service. Services will be provided once any outstanding balances are cleared. Our practice is cash-based and does not accept insurance, making clients responsible for the cost of treatment. We appreciate your cooperation and understanding.

  • We accept HSA/FSA payments.
