Step 1 of 2: COMPLETE YOUR INTAKE FORMS

We require completing your intake paperwork within 24 hours of securing your intake date for injury management and 72 hours for neurologic/concussion cases so Dr. Burkhardt can review and prepare. If the doctor sees something that leads them to believe he can't help you, you are eligible for a full refund. We tried our best to limit the number of repeat questions: some purposeful redundancy is built-in for thoroughness. 

 

For neurological or concussion care, use the "Neuro" button. For treatment for any other injury or maintenance care, please use the "Standard" button.

Step 2 of 2: SECURE YOUR DATE

Appointment Booking and Policies:

  • Appointment Scheduling: Our practice operates on an appointment-only basis. Existing patients can book same-day appointments subject to availability, which can be arranged online with a minimum of 2 hours notice. If no appointments are available for your chosen service,  they are booked.

​

  • New Patients: If you are new, returning after two years, or have a new injury, please select "New Patient Exam" when booking. New patients must submit all required paperwork at least 24 hours before their appointment, or 72 hours for neuro/concussion cases. Incomplete paperwork will result in cancellation and forfeiture of your deposit.

​

  • Punctuality: Please arrive on time. Arrivals more than 5 minutes late may have their appointment canceled to maintain service quality and respect other clients' time.

​

  • Cancellation Policy: Notify us at least 24 hours in advance to cancel or reschedule your appointment, or 72 hours for neuro/concussion treatments, to avoid a $50 cancellation fee or deduction from your treatment package. Changes can be made through the link in your booking confirmation email or text.

  • Treatment Room: Only the client and the doctor are allowed in the treatment room during appointments.

​

  • Payment: Payment is due at the time of service. We are a cash-based practice and do not accept insurance, making clients responsible for the cost of treatment. We accept HSA/FSA payments.

​
